I agree as a legally-binding company representative to the "Employment Screening Services Agreement" that follows:

Employment Screening Services Agreement The undersigned, desiring to use your pre employment screening service, agrees that all reports requested and received will be subjected to the following conditions: The undersigned agrees under the Fair Credit Reporting Act Section 604(a)(3)(B), 604(b) and other applicable laws, the requested forms will be used for employment purpose and where the prospective employee or employee has given written permission. (For employment purpose is defined as hiring and promotional decisions.) The undersigned agrees that all reports will be kept strictly confidential. Except as required by law, no information from reports will be revealed to the person reported on or to any other person with the exception of a person whose duty requires him or her to review the information in relation to which the report was intended. The undersigned agrees that in all instances where a report is ordered, he or she has in their possession a signed release from the prospective employee or employee. The signed release from the prospective employee or employee will indicate his or he agreement without reservation, to a search of his or her background in the areas of but not limited to his or her credit history, criminal history, civil court history, workers compensation history, driving history, educational history, personal references and previous employment history. The signed release from the prospective employee or employee will also contain his or her agreement without reservation to indemnify and hold harmless, Squire Protection Service, Inc., its employees or agents from any costs or liabilities in connection with or resulting from these reports. The undersigned agrees that they will comply with any applicable Federal, State or Local Laws or requirements concerning access to or use of said reports. In this regard, we will cooperate with you in monitoring our use of such reports. The undersigned agrees to indemnify and hold eBackground-Check.com and its affiliated companies and their affiliates from whom you obtain these reports and your officers and their officers, agents, employees and independent contractors harmless from and against any and all damages resulting from the publishing or use by us, our employees or agents of report information contrary to these conditions. The undersigned recognizing and agreeing that the requested information is secured by and through fallible human sources and you cannot be an insurer as to the accuracy of the information. We understand and agree that the accuracy of the information is not guaranteed by you and we agree to indemnify and hold harmless, you and your affiliated companies, any companies and their affiliates from whom you obtain these reports ad your and their officers, agents employees and independent contractors from any liability for any negligence, record content, errors or omissions in connection with the preparation of these reports. The undersigned agrees that they will pay upon receipt, all bills for services according to your rate schedule in effect at the time such services were rendered. eBackground-Check.com's Subscription Service requires an initial term of one year from the date of sign up. By ordering a subscription service, the undersigned subscriber hereby authorizes eBackground-Check.com and/or its agents to charge the subscriber's credit card each month for the subscriber fees. The undersigned agrees to maintain a valid credit card on file with eBackground-Check.com.
